Shipping Samples to Us |

1. All total RNA and DNA samples should be sent frozen on dry ice in an appropriate container. 2. All sample tubes should be clearly labeled (top and side), tightly closed and placed in a small zip-lock bag (please remove excess air). Place the closed bag on top of coarsely ground, medium to large, pieces of dry ice, which fill half the volume of the insulated/foam container. 3. Once the sample bag has been added, fill the remaining interior portion of the insulated/foam container with medium to large pieces of dry ice to very near the top of the container. 4. Place the insulated/foam cover on top and then seal with packing tape all around the seam of the lid. 5. Place the sealed insulated/foam container inside a made to fit, heavy gage cardboard shipping box. (Note – if shipping Affymetrix arrays along with your samples, please use a slightly larger, heavy gage cardboard shipping box, large enough to hold both the foam container and the additional Affymetrix arrays). When including GeneChip arrays, place the foil-wrapped arrays inside a large zip-lock bag and then lay this sealed plastic bag on top of the closed insulated/foam container. DO NOT place the arrays inside the insulated/foam container with the samples as this will cause the GeneChip arrays to freeze during transit, thus ruining the arrays and rendering them potentially useless for downstream analysis. Fill any remaining space within the cardboard shipping box with loose packing material. 6. Place a completely filled out Affymetrix Order Form and a Hazardous Material Declaration Form inside the cardboard box. Close the box and seal with packing tape (Note – Samples received that do not have both an accompanying filled out/signed Affymetrix Order Form and a Hazardous Material Declaration Form will not be processed at all until receipt of both completed forms). 7. Address the package as required by your shipping service provider. Attach appropriate labels for your shipper indicating dry ice inside and send via a commercial overnight delivery service (FedEx or United Parcel), selecting overnight priority-next morning delivery as the mode of choice. 8. Please e-mail or call your contact at the Facility and notify them the day you have shipped your samples to us. Please provide us with your shipping vendor’s name and parcel’s tracking number. In return, we will notify you upon the safe arrival of your samples and/or arrays. |
