Hazardous Material Declaration
This Declaration Form must be completely filled-out, signed and included with any samples conveyed (to include both shipped or hand carried) to the Facility. We have instigated this policy in order to provide our Facility staff with increased health and safety precaution as they come in direct contact with the provided samples during the course of unpacking, handling, transferring and subsequent processing. Please be aware that any provided samples lacking this required Declaration Form will not be processed by the staff and may be subject to disposal without receipt of this completed Form.
