Guidelines Form
All non-commercial new users of our Facility are required to download and print out a copy of our Guidelines Form, have it signed by the principal investigator of their laboratory and then return/send the signed copy to the UCI DNA & Protein MicroArray Facility. Only after receipt of this signed Guidelines Form will the Facility Staff be able to assist the members of the laboratory requesting our services. This Form only needs to be signed and sent once for our records. If you like you may enclose this signed Form with your initial sample set submission.
