Affymetrix Order Form
All users requesting Affymetrix based expression array services from our Facility are required to download, print out and completely fill in a copy of our Affymetrix Order Form each time they submit samples to our Facility. Please place this completed Form within the shipping package if sending samples to our Facility for processing. If you have questions concerning filling in the Form or requested services please check with the Facility staff (telephone or e-mail) prior to sending/providing the samples.
